Register through CFT!

We now have two ways to register. You can register online using the form below, or you can print out an enrollment form and fax it to our office. To enroll using a paper copy, follow these few steps:

Print out the enrollment form;
Fill it out and sign the form;
Have the form signed by a manager;
Fax the form to the CFT office: 904-354-1834

Registrations must be received no later than one week prior to the start date.
